A critical appraisal of the literature on Palatal Plate Therapy for orofacial dysfunction in patients with Down syndrome

Abstract - Orofacial regulation therapy (OFRT) was developed to improve orofacial function in patients with hypotonic oral musculature. Aim and objectives: To examine the current evidence on the use of Palatal Plate Therapy by dentists in patients with Down syndrome and determine if there is a role for dentists in the multidisciplinary team managing orofacial dysfunction and if further research is necessary. Methodology: Articles were sourced from 1990 onwards and those fulfilling the selection criteria were critically appraised by one reviewer and discussed in depth with two experienced reviewers. Results: There were limitations in the data collection due to study design and patient factors, as well as difficulties in obtaining papers describing randomised control trials with single variables. Limitations included inconsistent baseline data in the studies reviewed. Long term outcomes were difficult to assess as a result of high dropout rates in several studies. Conclusions: The results inferred a significant positive impact on patient’s lives with Down syndrome. The review suggests that larger multicentre research is required, in relation to orofacial regulation therapy, to obtain more statistically significant and valid results. Standardised objectives are required, as well as clear inclusion and exclusion criteria using reproducible methodology to gain meaningful results that may improve the quality of life for people with Down syndrome.
