The impact of congenital alkaline phosphatase deficiency on oral health condition: a literature review

Abstract Deficiency of alkaline phosphatase enzyme (ALP) is linked to an inherited disorder described as hypophosphatasia (HPP), which is associated with disturbances in bone and tooth mineralisation, resulting in various degrees of skeletal and dental malformations. The disease occurs due to mutations in the alkaline phosphatase gene which regulates the development of specific tissue. There is a remarkable heterogeneity in symptom occurrence depending on age at initial onset of disease, ranging from death in utero to relatively simple problems with dentition in adult life. Characteristic dental symptoms of HPP include premature loss of deciduous teeth, premature exfoliation of fully rooted permanent teeth, and alveolar bone loss. The aim of this paper is to demonstrate the various influences of low level of ALP with special reference to the dental aspects of the condition, based on an up-to-date review of the available systematic research and literature. Unexplained early tooth loss in children or adults may have a systemic cause and may be a feature of rare conditions such as HPP. As premature loss of primary teeth is often the first, and sometimes the only visible symptom of the milder phenotypes of HPP, dental practitioners can play a vital role in the early diagnosis, in dental management and in provision of preventive measures.
