Oral health status and oral health related quality of life in patients with congenital bleeding disorders

Abstract Introduction: There is a lack of information in the literature on the oral health status and its impact on the quality of life in adult patients with congenital bleeding disorders. Some evidence suggests that bleeding, transfusion transmitted infection and the development of inhibitors can impact on oral health and provision of dental care in this group of patients. Aim: To establish the oral health status of patients with the congenital bleeding disorders of Haemophilia A, Haemophilia B and von Willebrand disease and to determine if oral health status impacts on quality of life in this group of patients. Method: A convenience sample of 18 patients underwent a standardised clinical dental examination and completed a structured questionnaire incorporating the Oral Health Impact Profile-14 instrument and a Visual Analogue Scale to measure self assessed likelihood of having dental disease. Results: In this convenience sample of 18 patients, visible plaque, untreated dental caries, calculus and bleeding on probing were all higher than in age matched dentate adults in England, Wales and Northern Ireland in 2009. The results suggest that oral health has a greater impact on the quality of life of the sample population compared with adults in England, Wales and Northern Ireland in 2009. Conclusions and recommendations: Oral health and oral health related quality of life is poorer amongst individuals with congenital bleeding disorders. Suggestions are made for the development of care pathways and educational approaches to address this inequality.
