The changing face of an adult with special needs- treatment planning considerations for dental management under general anaesthesia

Patients with special needs often present with multiple medical comorbidities. With advances in medical management, these individuals enjoy much improved survival times but also carry significant medical and surgical risks associated with invasive procedures under general anaesthesia(GA). A new cohort of patients is emerging: adult patients with cerebral palsy who face ongoing aspiration risk; adult survivors of childhood haematological malignancies who
carry the adverse dental effects associated with radiotherapy and graft vs host disease following bone marrow transplantation; individuals with long-term complications of corticosteroid therapy including osteoporosis managed with
bisphosphonate therapy; obese individuals with special needs who are not compliant for comprehensive assessment and
management of obstructive sleep apnoea; and non-mobile individuals who are transferred from Coumadin to NOAC
anti-coagulant therapy.
