P1 Interdisciplinary treatment of hypodontia

Most frequently this causes single tooth agenesis in the lower second premolars and upper lateral incisors. Congenitally missing maxillary lateral incisors create an aesthetic problem but also induce the development of malocclusion. Unilaterally missing teeth additionally causes dental arch asymmetry. The aim of this work was to present results of interdisciplinary management of congenitally missing maxillary lateral incisors with application of intraosseous dental implants. Case 1: 20-year-old patient with both upper lateral incisors congenitally missing treated for lateral crossbite on the right side. Orthodontic treatment with fixed appliance protocol was planned to create the space for intraosseous implants. After sixteen months of treatment good occlusion and proper bone conditions for Osteoplant implants (diameter 3.5mm, length 12mm) application was obtained and ceramic crowns were made for both upper lateral incisors. Case 2: 24-year-old patient with absent maxillary left incisor and partial agenesis of the right maxillary incisor. First phase of treatment dealt with orthodontic fixed appliance for proper tooth alignment in the upper dental arch and space opening for intraosseous implant. Osteoplant implant (diameter 3.5mm, length 12mm) of missing upper lateral incisor was placed. Three months after implantation, ceramic crowns were made for both upper lateral incisors. The interdisciplinary orthodontic-surgical-prosthetic treatment of congenitally missing teeth enables reconstruction of normal occlusion and improves facial appearance.

P3 Oral rehabilitation with mini-implant-supported overdenture in a child with ectodermal dysplasia syndrome (ED)

M Montanari, F Battelli, G Piana, Department of Oral Science Alma Mater Studiorum University of Bologna, Italy. montmarco@virgilio.it Ectodermal dysplasias are a heterogeneous group of inherited diseases characterised by abnormal development of ectodermal origin tissues. An affected 3-year-old male showed hypotrichosis, nail dystrophia, frontal bossing, prominent lips, depressed midface, thin skin, hypo-anhydrosis, no dental elements, dry and sticky oral mucosa and lack of the alveolar ridge. Anodontia was confirmed by orthopantomography. OligoAnodontia affected chewing, speech, aesthetics and social relations. Early prosthetic rehabilitation is essential to improve oral function and reduce social impairment. Reduced retention is one of the most important problems of lower dentures therefore, two 2.2 x 6mm miniimplants were placed in the canine region of the anterior mandible. After a submerged healing period of 4 months the implants were exposed and abutment connection was performed. A ball attachment-based denture was constructed with a double cylindrical expanding guide to permit physiological jaw growth. The child was monitored clinically every month to fit prostheses and evaluate masticatory function. Cephalometric radiographs were taken every 6 months with the prostheses in place. A significant improvement in speech, masticatory function, and facial aesthetics was reported. There was no evidence that implant rehabilitation resulted in restriction of transverse or sagittal growth thanks to the double cylindrical expanding guide inserted. The study suggests that mini-implants can be successfully placed in atrophic jaws of children and provide support for prosthetic restoration.

Radiation therapy is often applied preceding or following cancer surgery to improve its outcome. The development of surgical techniques, such as dental implants, may be useful in the rehabilitation of this compromised patient. Prospective studies reporting on the treatment outcome of implant-retained overdentures in healthy patients suffering from impaired oral functioning due to an instable lower denture have noted that implant-retained overdentures have proven to be a reliable treatment for problems involving lack of stability and retention of a lower denture. This high success rate means that a similar prosthodontic treatment approach may help to achieve better functional results in the oral rehabilitation of head–neck cancer patients. The aim of this study is to report a case in which a patient has been rehabilitated with an implant supported overdenture after radiation therapy.

The use of facial prostheses is a current solution for such problems. Even so, conventional prostheses depend on retention by additional devices, which sometimes do not improve the patient’s quality of life. Using titanium implants to attach the prosthesis started many years ago, improving retention and patient’s quality of life, although because of the costs and complications, its regular use is very controversial in the literature. The purpose of this presentation is to display a case of rehabilitation after trauma resection of the ear. Furthermore, to discuss the indications and advantages using osseointegrated implants as well as the rehabilitation process in these facial rehabilitation cases. The ear prosthesis was made of silicone and attached on three extra-oral implants with treated surfaces. A significant improvement of this patient’s aesthetics was achieved by this method with enormous increase of self-esteem.
